Clinical study of tesamorelin has centered on adults with HIV-associated lipodystrophy, a condition in which abdominal fat accumulates while peripheral fat is lost. In controlled trials, treated participants showed reductions in visceral adipose tissue measured by imaging, alongside modest shifts in some lipid values. Effects on subcutaneous fat were smaller and less consistent across studies. Whether these changes translate into fewer cardiovascular events remains an open question, because the trials were not designed or powered to answer it.
Tesamorelin is a synthetic peptide that acts as an analog of growth hormone-releasing hormone, a natural hypothalamic signal. Its sequence corresponds to the forty-four amino acid form of the human hormone, with a small acyl group attached near the amino terminus. That modification slows enzymatic breakdown and extends the time the peptide remains active in circulation. The compound was developed as a pharmacological way to raise endogenous growth hormone output rather than supplying the hormone directly.
After injection, the peptide binds receptors on somatotroph cells in the anterior pituitary. Receptor activation raises intracellular cyclic AMP and triggers release of stored growth hormone into the bloodstream. Because the compound works through the body's own regulatory system, growth hormone pulses retain much of their normal feedback control. Repeated administration also raises insulin-like growth factor 1, a hormone produced mainly in the liver. Investigators treat that rise as a marker that the pituitary axis has been engaged.
Studies of the compound rely on imaging and laboratory endpoints rather than on symptoms alone. Visceral adipose tissue is usually quantified by computed tomography or magnetic resonance imaging at the level of the abdomen, with waist circumference serving as a cheaper but less specific proxy. Blood work tracks insulin-like growth factor 1, fasting glucose, glycated hemoglobin, and lipid fractions. In the pivotal trials the imaging endpoint fell by roughly fifteen to twenty percent over six months, subcutaneous fat changed little, and the visceral fat returned toward baseline after treatment stopped, a pattern that shapes how clinicians discuss durability.
Whether the drug improves hard clinical outcomes is not settled. No completed trial has shown a reduction in heart attacks or strokes among treated patients, although a dedicated cardiovascular outcomes study has been discussed in the literature. Investigators have also examined hepatic fat in people with HIV and fatty liver disease, cognitive measures in small cohorts, and changes in bone density. Regulatory labeling emphasizes monitoring of insulin-like growth factor 1 because supraphysiologic levels raise questions about tissue growth, and the clinical significance of that signal remains an open question rather than a demonstrated harm.
Binding of tesamorelin to the growth hormone-releasing hormone receptor on anterior pituitary somatotrophs activates a Gs protein pathway, raises cyclic AMP, and triggers release of stored growth hormone into the bloodstream. Because the analogue resists dipeptidyl peptidase-4, its plasma residence time exceeds that of native GHRH, producing a larger and more sustained secretory signal. The released growth hormone then acts on the liver and peripheral tissues to raise insulin-like growth factor 1, which feeds back on the hypothalamus and pituitary. This axis explains both the intended effects on fat distribution and the biological markers used to track them.

The native hormone is produced in the hypothalamus and acts on the anterior pituitary. Binding of GHRH to its receptor stimulates synthesis and release of growth hormone into circulation. Because the analogue retains the receptor-binding region of the parent sequence, it engages the same receptor and triggers the same downstream signaling. The result is increased growth hormone secretion from pituitary cells, which in turn influences hepatic production of insulin-like growth factor 1. This axis is the basis for the compound's measured biological effects.
Interest in this peptide developed because native GHRH has a short circulating lifetime. The N-terminal modification slows cleavage by dipeptidyl peptidase IV, an enzyme that removes the first two residues of many peptides and terminates their activity. Slower degradation means a longer window of receptor stimulation per administration. This design logic parallels other modified peptide hormones, where a small chemical change at a vulnerable site yields a more durable molecule without altering the core mechanism of action.
Practical handling centers on limiting moisture, oxygen, and temperature excursions. Lyophilized material is generally held at or below minus twenty degrees Celsius, protected from light and kept sealed until use. Once reconstituted, solutions are typically kept cold and used within a short window because hydrolysis and microbial growth both accelerate in liquid form. Repeated freeze-thaw cycles are avoided, since they promote aggregation. Vial contents should be inspected for particulates and clarity before analysis, and working aliquots are prepared to reduce the number of times the stock is opened.
Quantitation of the peptide relies mainly on reversed-phase high-performance liquid chromatography with ultraviolet detection, typically at 214 nanometers, where the peptide bond absorbs. Identity is confirmed by mass spectrometry, most often electrospray ionization coupled to liquid chromatography, and by peptide mapping after enzymatic digestion. Because related impurities differ only slightly in sequence or modification, method development emphasizes resolution rather than speed. Purity is usually reported as a percentage of the main peak area, with individual impurities listed separately when they exceed a defined reporting threshold.
Development work on the compound, originally designated TH9507, focused on conditions in which reduced growth hormone signaling is thought to contribute to altered body composition. The United States Food and Drug Administration approved it in 2010 for the treatment of excess visceral abdominal fat in adults with human immunodeficiency virus infection and lipodystrophy. Later research examined other populations, including adults with mild cognitive impairment, where a large trial did not meet its primary endpoints. This mixed record illustrates how a single mechanism can produce clear effects in one setting and inconclusive results in another.
Several related peptides act on the same receptor, including sermorelin, a shorter GHRH fragment, and modified analogs such as CJC-1295 and modified GRF(1-29) that are common in research settings rather than approved products. Tesamorelin differs from growth hormone itself in that it acts upstream, prompting the pituitary to release the hormone through physiological signaling rather than supplying it directly. Terminology in the literature distinguishes GHRH analogs, growth hormone secretagogues, and recombinant growth hormone, although popular discussion often blurs these categories together. Precise naming matters when comparing study results.
Tesamorelin is a synthetic peptide of 44 amino acids that reproduces the sequence of human growth hormone-releasing hormone (GHRH) and carries a trans-3-hexenoyl group on its N-terminal tyrosine. That small fatty-acid modification blocks cleavage by dipeptidyl peptidase-4, the enzyme that rapidly degrades native GHRH in plasma. The result is a molecule with a longer circulating half-life than the natural hormone while retaining the same receptor target. It is supplied as a lyophilized powder for reconstitution and belongs to the broader class of GHRH analogs studied for effects on pituitary growth hormone secretion.
The Volkshalle (German pronunciation: [ˈfɔlksˌhalə], "People's Hall"), also called Große Halle ([ˌɡʁoːsə ˈhalə], "Great Hall") or Ruhmeshalle ([ˈʁuːməsˌhalə], "Hall of Glory"), was a proposal for a monumental, domed building to be built in a reconstituted Berlin (renamed as Germania) in Nazi Germany. The project was conceived by Adolf Hitler and designed by his architect Albert Speer. No part of the building was ever constructed. The word Volk had a particular resonance in Nazi thinking. The term völkisch movement, which can be translated to English as "the people's movement" or "the folkish movement", derives from Volk but also implies a particularly racial undertone. Before the First World War, völkisch thought had developed an attitude to the arts as the German Volk; that is, from an organically linked Aryan or Nordic community (Volksgemeinschaft), racially unpolluted and with its roots in the German soil of the Heimat (homeland).
== Epidemiology == Many over-the-counter and prescription-only medications contain paracetamol. Because of its wide availability paired with comparably high toxicity, (compared to ibuprofen and aspirin) there is a much higher potential for overdose. Paracetamol toxicity is one of the most common causes of poisoning worldwide. In the United States, the United Kingdom, Australia, and New Zealand, paracetamol is the most common cause of drug overdoses. Additionally, in both the United States and the United Kingdom it is the most common cause of acute liver failure. In England and Wales an estimated 41,200 cases of paracetamol poisoning occurred in 1989 to 1990, with a mortality of 0.40%. It is estimated that 150 to 200 deaths and 15 to 20 liver transplants occur as a result of poisoning each year in England and Wales. Paracetamol overdose results in more calls to poison control centers in the US than overdose of any other pharmacological substance, accounting for more than 100,000 calls, as well as 56,000 emergency room visits, 2,600 hospitalizations, and 458 deaths due to acute liver failure per year. A study of cases of acute liver failure between November 2000 and October 2004 by the Centers for Disease Control and Prevention in the US found that paracetamol was the cause of 41% of all cases in adults, and 25% of cases in children.
Sodium oxybate has also been investigated and been of interest to improve sleep and associated symptoms in other conditions with sleep disruption, such as myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS) and long COVID, which also have high rates of non-restorative sleep. In addition, sodium oxybate was limitedly studied to improve insomnia in people with depression or bipolar disorder. However, it was reported to paradoxically disrupt sleep and induce narcolepsy-like changes in these individuals. Moreover, concerns about misuse have limited use of sodium oxybate for other medical conditions. GHB has also garnered a reputation as a date-rape drug, although the actual prevalence of this appears to be much lower than popular perception. The GABAB receptor agonist baclofen has also been more limitedly investigated for improvement of sleep and has been found to be effective in enhancing sleep similarly to sodium oxybate. However, in people with narcolepsy, baclofen and sodium oxybate both improved sleep but only sodium oxybate reduced daytime sleepiness. In any case, research in this area is limited, and there remains significant interest in baclofen in the potential treatment of sleeping problems. Unlike sodium oxybate, baclofen is not a controlled substance and has much less or no misuse potential. Baclofen and sodium oxybate have been found to activate the GABAB receptor differently, which is thought to underlie the differences in their effects.
Chlorination occurs at the amino acyl-PCP level during the biosynthesis, prior to phenolic oxidative coupling, with the possibility of tyrosine or β-hydroxytyrosine being the substrate of chlorination. Hydroxylation of the tyrosine residue of module 6 also occurs in trans during the assembly of the heptapeptide backbone.

Fabomotizole (INN), sold under the brand name Afobazole, is an anxiolytic drug launched in Russia in the early 2000s. It produces anxiolytic and neuroprotective effects without any sedative or muscle relaxant actions. Its mechanism of action remains poorly defined however, with GABAergic, NGF- and BDNF-release-promoting, MT1 receptor agonism, MT3 receptor antagonism, and sigma agonism suggested as potential mechanisms. Fabomotizole was shown to inhibit MAO-A reversibly and may potentially have some involvement with serotonin receptors. Clinical trials have shown fabomotizole to be well tolerated and reasonably effective for the treatment of anxiety. Experiments in mice have shown antimutagenic and antiteratogenic properties. Experiments in rats have shown beneficial effect in the model of ischemic stroke. Fabomotizole has found little clinical use outside Russia and has not been evaluated by the FDA.
In the first total synthesis in 1936, ethyl 3-ethoxypropanoate was treated with ethyl formate to give an intermediate dicarbonyl compound which when reacted with acetamidine formed a substituted pyrimidine. Conversion of its hydroxyl group to an amino group was carried out by nucleophilic aromatic substitution, first to the chloride derivative using phosphorus oxychloride, followed by treatment with ammonia. The ethoxy group was then converted to a bromo derivative using hydrobromic acid. In the final stage, thiamine (as its dibromide salt) was formed in an alkylation reaction using 4-methyl-5-(2-hydroxyethyl)thiazole.
As of 2013, the existence of efficient high-throughput next-generation sequencing technology allows for the identification of cause many different human disorders. Simple Mendelian inheritance has been observed for over 3,000 disorders that have been identified at the Online Mendelian Inheritance in Man database, but complex diseases are more difficult. Association studies have found many individual genetic regions that individually are weakly associated with complex diseases (such as infertility, breast cancer and Alzheimer's disease), rather than a single cause. There are currently many challenges to using genes for diagnosis and treatment, such as how we don't know which genes are important, or how stable the choices an algorithm provides. Genome-wide association studies have successfully identified thousands of common genetic variants for complex diseases and traits; however, these common variants only explain a small fraction of heritability. Rare variants may account for some of the missing heritability. Large-scale whole genome sequencing studies have rapidly sequenced millions of whole genomes, and such studies have identified hundreds of millions of rare variants. Functional annotations predict the effect or function of a genetic variant and help to prioritize rare functional variants, and incorporating these annotations can effectively boost the power of genetic association of rare variants analysis of whole genome sequencing studies.
Le Havre is a major port city in the Seine-Maritime department in the Normandy region of northern France. It is situated on the right bank of the estuary of the river Seine on the English Channel southwest of the Pays de Caux, very close to the Prime Meridian. Le Havre is the busiest port on the northern French Coast and largest container port in France. it is the second largest city in the Normandy region of France with total population of the greater Le Havre conurbation being smaller than that of Rouen. It is also the second largest subprefecture in France, after only Reims. The name Le Havre means 'the harbour' or 'the port'. Its inhabitants are known as Havrais (masculine) or Havraises (feminine). The city and port were founded by King Francis I in 1517. Economic development in the early modern period was hampered by religious wars, conflicts with the English, epidemics, and storms. It was from the end of the 18th century that Le Havre started growing and the port took off first with the slave trade then other international trade. After the 1944 bombings the firm of Auguste Perret began to rebuild the city in concrete. The oil, chemical, and automotive industries were dynamic during the Trente Glorieuses (postwar boom) but the 1970s marked the end of the golden age of ocean liners and the beginning of the economic crisis: the population declined, unemployment increased and remains at a high level today. Changes in years 1990–2000 were numerous.
The bacterial one-hybrid (B1H) system is a method for identifying the sequence-specific target site of a DNA-binding domain. In this system, a given transcription factor (TF) is expressed as a fusion to a subunit of RNA polymerase. In parallel, a library of randomized oligonucleotides representing potential TF target sequences are cloned into a separate vector containing the selectable genes HIS3 and URA3. If the DNA-binding domain (bait) binds a potential DNA target site (prey) in vivo, it will recruit RNA polymerase to the promoter and activate transcription of the reporter genes in that clone. The two reporter genes, HIS3 and URA3, allow for positive and negative selections, respectively. At the end of the process, positive clones are sequenced and examined with motif-finding tools in order to resolve the favoured DNA target sequence.

The tags contain four regions, namely a mass reporter region (M), a cleavable linker region (F), a mass normalization region (N) and a protein reactive group (R). The chemical structures of all the tags are identical but each contains isotopes substituted at various positions, such that the mass reporter and mass normalization regions have different molecular masses in each tag. The combined M-F-N-R regions of the tags have the same total molecular weights and structure so that during chromatographic or electrophoretic separation and in single MS mode, molecules labelled with different tags are indistinguishable. Upon fragmentation in MS/MS mode, sequence information is obtained from fragmentation of the peptide back bone and quantification data are simultaneously obtained from fragmentation of the tags, giving rise to mass reporter ions.
=== 9 May === Two people were killed by Russian shelling in Nikopol. In Russia, the governor of Belgorod Oblast claimed that eight people were injured in a Ukrainian airstrike in Belgorod city. A drone attack on Krasnodar also set a fuel depot on fire. A drone strike was also reported in an oil refinery in Salavat in the first such incident of its kind in the constituent republic of Bashkortostan. The drone used in the attack on Bashkiria was conducted by a drone light aircraft, make unknown, that travelled some 1,500 km. The SBU arrested a resident of Kharkiv on suspicion of spying for Russia. It also claimed to have arrested several individuals in Kyiv on suspicion of attempting to set off bombs on behalf of Russia. Zelenskyy implemented a number of changes in the Ukrainian defense apparatus. He appointed Brigadier General Oleksandr Trepak as commander of the Ukrainian Special Operations Forces, replacing Colonel Serhii Lupanchuk. He also dismissed Valeriy Zaluzhnyi, former commander-in-chief of Ukraine's Armed Forces, from military service "on health grounds", and reappointed Dmytro Hereha as the commander of the Ukrainian Support Forces after replacing him with Oleksandr Yakovets in March 2024. Zelenskyy also dismissed Serhii Rud as director of the State Security Administration. Two colonels were also arrested after being accused of being agents for the Russian FSB. The US Army announced that it would supply three additional HIMARS units paid for by Germany to Ukraine.
For example, in Florida, a MLT may only perform highly complex testing while under the direct supervision of a clinical laboratory technologist, a clinical laboratory supervisor, or a clinical laboratory director. This may make it impractical for a MLT to lawfully work in a Florida blood bank. California has similar restrictions on MLTs. To accommodate California's restrictions, the American Association of Bioanalysts (AAB) developed a separate certification examination for California licensure. However, this exam does not include material covering the areas of immunohematology or microscopy. Although the typical entry-level academic requirement for most MLTs is an associate degree, a 60 credit certificate program exists through military training programs; such as the U.S. Army's 68K military occupational specialty. As in other countries, staffing shortages have become a major issue in many clinical laboratories in the United States. Due to several factors, including boomer retirement, and inadequate recruitment and retention efforts, the medical laboratory workforce is shrinking. For the decade 2010–2020, workforce needs are expected to grow by 13%. This translates into about 11,300 positions per year that will need to be filled, with only about 5000 new graduates per year coming out of various programs. It was estimated that the shortage of medical laboratory professionals would reach 98,700 in the U.S. by 2025.
=== Barriers to access === In the US, the list price for a long-acting injectable form is five to 20 times as much as a daily pill. This has reduced the number of people who are able to get a single monthly dose, instead of daily pills. Some jails consider the more expensive form a positive tradeoff: a single monthly injection may be simpler and easier for the staff to manage than daily trips to the dispensary to have a nurse provide a pill and make sure that it has been swallowed.
== Hosts == D. dadantii causes disease on several different ornamental and horticultural host plants throughout the world including: tropical, subtropical, and temperate climates. The host range of D. dadantii continues growing as new susceptible species are continuously being documented. It has also been found in soils, rivers and irrigation water. Host specificity is not yet fully understood. Originally pathovar groups were documented according to the hosts from which they were isolated. Today 50+ species have been identified and more are possible if another classification system based on biovars were to be used. Disease is most often reported on bananas, carnations, and chrysanthemums, but the list of host species is quite vast. Important host families and species economically affected include:

It is a laboratory-made peptide that mimics growth hormone-releasing hormone. It prompts the pituitary gland to release growth hormone and has been studied mainly in adults with HIV-associated lipodystrophy.
Administered growth hormone supplies the hormone directly, while this peptide acts upstream by prompting the pituitary to release it. The indirect route preserves pulsatile secretion and some endogenous feedback, which changes the hormone and IGF-1 profile observed after treatment.
Reductions in visceral adipose tissue appear consistently in randomized trials of the approved population. Effects on peripheral fat, cardiovascular outcomes, and use outside that population are less well established.
It mimics a natural hypothalamic signal that tells the pituitary to release growth hormone. The result is a rise in circulating growth hormone and, indirectly, in insulin-like growth factor 1. Over weeks of treatment this shift is associated with a selective decrease in fat stored inside the abdomen.
